In medical aesthetics, “filler” is often used as a blanket term. But if you want meaningful, long-term rejuvenation, it helps to understand that not all injectables work the same way.

At The Vanity Lab, we group volume-restoring treatments into two categories:

  1. Traditional Hyaluronic Acid (HA) Fillers
  2. Biostimulators (Sculptra® and Radiesse®)

Both improve volume loss and soften lines, but they do it through very different mechanisms:

  • One gives you instant lift
  • The other rebuilds your skin’s foundation over time

Here’s how they work, and most importantly, how to choose the option that aligns with your unique goals.

How Dermal Fillers & Biostimulators Work

Traditional Dermal Fillers (The "Instant Fix")

HA fillers (such as Juvederm® and Restylane®) act as a smooth, gel-like cushion beneath the skin. They physically fill space, instantly plumping lips, restoring contour, or softening etched lines. Hyaluronic Acid is naturally present in the skin and attracts water, which helps maintain hydration and structure.

Dermal fillers are best for: Immediate results, lip augmentation, feature enhancement, fine-tuning specific areas.

Biostimulators: Sculptra® & Radiesse® (The "Foundation Builders")

Biostimulators don’t behave like classic fillers. Instead of sitting where they’re placed, they trigger your body’s collagen-building response.

  • Sculptra® is made from poly-L-lactic acid (PLLA), which encourages the skin to gradually rebuild its support structure.
  • Radiesse® uses calcium hydroxyapatite (CaHA) microspheres. These provide immediate scaffolding while stimulating collagen and elastin production over time.

Biostimulators are best for: Natural, progressive improvements in volume, contour, and skin quality.

Comparing the Two: What to Expect

Since there is no "one size fits all" in medical aesthetics, understanding the key differences between these two treatments will help you decide which route to take.

1. Timeline of Results

  • Traditional Fillers: These offer immediate gratification. If you want to walk out of the clinic with fuller lips or sharper cheekbones instantly, traditional Hyaluronic Acid (HA) fillers are the way to go.
  • Biostimulators (Sculptra® & Radiesse®): These require patience. While you may see some immediate fullness due to swelling or the carrier gel , this typically dissipates within 7–14 days. The "real" results appear gradually as your body produces new collagen, with the best visible changes occurring around 12 weeks post-treatment.

2. Longevity

  • Traditional Fillers: Depending on the product and your metabolism, HA fillers typically last between 6 to 12 months before the body naturally breaks them down.
  • Biostimulators: Because these products rebuild the structural foundation of your skin, the results are significantly longer-lasting. You can expect your results to last up to 2 years.

3. The Mechanism

  • Traditional Fillers: These work by physically filling physical space. They act like a cushion under the skin to smooth wrinkles or add volume.
  • Biostimulators: These are "bio-active." They trigger a controlled inflammatory response in the skin. This signals your body to produce its own collagen and elastin, effectively reversing the signs of aging from the inside out.

4. Aftercare and Downtime

  • Traditional Fillers: The downtime is generally minimal, with some potential bruising or swelling.
  • Biostimulators: You can expect swelling for about a week or two. Crucially, products like Sculptra® require "homework." To prevent nodules and ensure even distribution, you must massage the treated area for 5 minutes, 5 times a day, for 5 days.


When to Choose Biostimulators (Sculptra or Radiesse)

You should consider a biostimulator treatment at our Vancouver clinic if you're noticing generalized volume loss or "hollows" in the cheeks and temples, and you prefer a natural, gradual transition over an overnight change.

1. You Want Long-Term, Natural-Looking Results

Biostimulators strengthen the skin’s structural foundation. Because your body is producing the collagen, the result looks and feels natural and lasts longer.

2. You Want to Treat Body Areas (Like Hip Dips)

Biostimulators work extremely well in areas with generalized volume loss, such as the temples, cheeks, or jawline, and can also be used for body contouring (e.g., hip dips).

3. You Committed to Aftercare

Success with Sculptra requires patient participation. If you’re willing and able to follow the aftercare instructions, you’ll likely see strong results.

When to Choose Traditional HA Fillers

Traditional fillers remain the gold standard for:

  • Lip Injections: Biostimulators are generally not used in the lips.
  • Under-Eye Rejuvenation (Tear Troughs): This delicate area often requires the soft, smooth consistency of HA gel.
  • Immediate Events: If you have a wedding or reunion in the coming days, HA fillers offer the instant "wow" factor you need.
